Responsibilities
- Assist individuals with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or resident’s
lifts. - Monitor patient health by observing vital signs, medication administration, and overall condition to ensure safety and well-being.
- Implement and follow individualized care plans tailored to each resident’s needs, including dementia care, Alzheimer’s care, memory care, and autism support.
- Prepare nutritious meals and assist with meal planning while maintaining cleanliness in the kitchen area.
- Maintain accurate documentation of patient care activities while adhering to HIPAA regulations to ensure confidentiality.
- Support residents during group activities and community outings to promote social engagement and independence.
- Perform light cleaning duties to uphold a safe and hygienic environment in accordance with health standards.
- Respond promptly to emergencies or behavioral challenges with patience and professionalism; provide first aid or CPR when necessary.
- Collaborate with healthcare professionals such as nurses or social workers to update care plans and ensure comprehensive support.
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Demonstrated knowledge of patient observation techniques, medication administration, and vital signs monitoring.
- Experience with dementia care, Alzheimer’s care, hospice care, or working with individuals with disabilities enhances your ability to provide specialized support.
- Familiarity with safety procedures including first aid, CPR certification, heavy lifting techniques, and resident transfer equipment like Hoyer lifts.
- Background in long-term care or home & community care settings that involve assisting with ADLs and patient monitoring is advantageous.
- Knowledge of HIPAA compliance ensures respectful handling of sensitive information related to patient privacy.
