Schedule & Coverage

• Schedule: Monday–Friday, 8:00 AM – 5:00 PM
• On-Call: None
• Territory: Guilford and Randolph counties
• Mileage Reimbursement: $0.43 per mile
• Productivity Requirement: 31.5 points per week

Job Summary

We are seeking an experienced Registered Nurse Case Manager to support high-risk members in a home health setting. This role focuses on comprehensive assessments, care coordination, and patient advocacy to ensure members receive the most appropriate and cost-effective care.

Primary Responsibilities

• Perform comprehensive assessments of high-risk members in the home health setting
• Collaborate with primary care providers to implement individualized, comprehensive care plans
• Continuously reassess and ensure appropriate, cost-effective levels of care
• Coordinate care transitions across healthcare settings
• Work closely with physicians, social workers, discharge planners, and claims professionals
• Arrange non-medical support services such as transportation and housing to ensure treatment compliance
• Engage specialty resources as needed to achieve optimal patient outcomes
• Maintain detailed documentation of clinical, functional, and financial outcomes
• Identify opportunities for health promotion and illness prevention
• Intervene quickly to prevent or minimize adverse patient outcomes
• Serve as a patient advocate while protecting privacy and confidentiality

Required Skills & Experience

• Home Health experience required
• Experience with Homecare Homebase (HCHB)
• OASIS documentation experience required
• Strong care coordination and clinical decision-making skills

Qualifications

• Associate’s or Bachelor’s degree in Nursing or related field
• Current, unrestricted Registered Nurse (RN) license
• North Carolina or Compact RN License required
• Case Management Certification preferred
• Ability to analyze complex medical information and make sound clinical decisions
