Job Overview
We are seeking a dedicated and compassionate LVN for our Home Health Pediatric client. The ideal candidate will provide high-quality nursing care to the client in his home, ensuring his comfort and well-being. This role requires a strong skill set in Pediatric Nursing, commitment to patient-centered care, excellent communication skills, and the ability to work independently while collaborating with a multidisciplinary team.
Responsibilities
- Provide care for a client with trach and G-tube as well as oral/bottle feedings three night (Fri, Sat, Sun) and two day shifts (Wed/Fri).
- Conduct comprehensive patient assessment and develop individualized care plan.
- Monitor patient's vital signs and overall health status, documenting changes accurately.
- Collaborate with healthcare professionals to coordinate care and ensure continuity of services.
- Maintain accurate medical records in compliance with HIPAA regulations and organizational policies.
- Participate in case management activities, including discharge planning and follow-up care.
- Utilize EMR systems effectively for documentation and patient management.
Skills
Candidates should possess a diverse skill set that includes but is not limited to:
- Proficiency in EMR systems.
- Experience with critical care and NICU is a plus.
- Knowledge of medical terminology, anatomy, physiology, and patient assessment techniques.
- Familiarity with accurate medical documentation.
- Competence in performing procedures such as G-tube feedings and trach suctioning.
- Experience in various healthcare settings including home care, hospice & palliative medicine, pediatrics, emergency medicine, internal medicine, and outpatient services.
- Understanding of Universal Precautions and infection control protocols.

Pay: $45.00 - $50.00 per hour
