What are the responsibilities and job description for the LVN / Client Concierge – Full-Time position at Heart of Gold Nursing?
We are looking for a compassionate, tech-savvy Licensed Vocational Nurse (LVN) to join our heart-centered senior care team as a Client Concierge. In this role, you’ll combine clinical expertise with relationship-driven care to support both clients and caregivers.
What You’ll Do
- Client Care & Relationship Management
- Conduct client supervisory visits to ensure exceptional, heartfelt care
- Connect with clients within 24 hours of starting care, with follow-ups at 1, 3, and 7 days, then ongoing monthly check-ins
- Build strong, trusting relationships with clients and their families through regular communication and thoughtful follow-up
- Address and resolve concerns in the home with empathy, professionalism, and a solutions-focused mindset
- Ensure every client is a great fit for our services and feels truly supported
- Care Planning & Coordination
- Create, update, and maintain personalized care plans reflecting each client’s unique needs
- Participate in bi-weekly Nurse Care Management (NCM) meetings and provide insights to the team
- Collaborate with outside providers to ensure clients receive seamless, well-rounded care
- Keep client records accurate and up to date in our database
- Provide caregivers with helpful client snapshots so they feel confident and prepared in the home
Qualifications & Skillset
- Active California LVN license required
- A natural “people person” with a strong hospitality mindset
- Warm, compassionate bedside manner that helps clients feel at ease
- Tech-savvy and comfortable using multiple systems
- Strong conflict resolution skills you stay calm, kind, and solution-focused
- Thrive in a fast-paced environment where no two days are the same
- Excellent time management and ability to juggle multiple priorities
- Open to participating in an on-call rotation (nights and weekends every 3 weeks)
