Responsible for maintaining all physical therapy, occupation therapy, speech therapy, and cardiac/pulmonary rehab records accurately and orderly and timely. Performs clerical and reception duties. Performs Rehab and Wellness Transporter, Therapy Aide, and Wellness Aide duties in their absence as directed.
TYPICAL PHYSICAL DEMANDS: Requires full range of body motion, standing and walking during majority of work hours. Must be able to lift 50 pounds and be able to pivot transfer 175 pounds safely. A significant amount of bending, lifting, and stretching is required. Good coordination and balance are a must. Requires corrected vision and hearing to functional range. Must possess good communication skills and possess a working knowledge of reading and writing. Requires working under stressful conditions at times and exposure to communicable diseases or bodily fluids.

- Mature, caring individual with good organizational skills and attention to detail.
- No specific experience is necessary, but previous medical experience is preferred.
- Must be in good physical condition.

- Be alert to all patients and clients AT ALL TIMES while they are in the Outpatient Wellness Center.
- Prepare, support, transfer, and stabilize patients to assist in administration of treatments.
- Safeguard, motivate, and assist patients during treatments.
- Observe patients for signs of distress or problems while in the department.
- Maintain a safe environment and condition for patients while in the department.
- Assist with care to patients of all ages with varying diagnosis (es) admitted to the Heart of America Medical Center.
- Maintain weekly inventory of supplies and assist with ordering as directed.
- Maintain the storage cupboards and closets orderly and neatly.
- Insure that the phone is manned and someone is attending the reception desk AT ALL TIMES.
- Perform the duties of the Wellness and Rehab Therapy Aide or Transporter in their absence.
- Able to identify, demonstrate and perform proper body mechanics and steps in performing standing pivot transfers and patient lifts.
- Demonstrate and perform proper procedures in patient lying, seating, and standing positioning.
- Maintain a safe, orderly, clean working environment in the department.
- In the event that it is your 7:00 AM day, turn on all lights, unlock all doors, open front door and all treatment rooms.
- In the event that it is your 5:00 PM day, straighten all the rooms and gym, remove all dirty linen to the area in which it is to be picked up by maintenance and empty all garbage to the area in which it is to be picked up by maintenance before you leave.
- Demonstrates an enthusiastic, motivating attitude and approach towards patients and staff while showing genuine respect and courtesy.
- Carries out directives in a positive manner showing self-initiative and accepts responsibility for follow-up.
- Is challenged by change, recognizes need for change, views constructive criticism as a tool for improving self as well as patient care.
- Communicates with supervisor and PT’s, OT”s, SLPs’ and Cardiac Rehab/Pulmonary Nurse on specific patient documentation, problems, etc in a timely manner and directs concerns to the appropriate individual(s), avoiding third part involvement and gossip.
- Familiar with and adheres to facility and departmental policies and procedures and continuous quality improvement programs.
- Seeks in-service/continuing education programs and participates in all mandatory facility and departmental IDM’s, education and training sessions.
- Maintain all medical records accurately, orderly and timely.
- Send and track outgoing medial records, primarily plan of treatments for physicians’ signatures, or others as necessary.
- Make copies of medical records as needed to complete documentation requirements.
- Supervise and complete the monthly Time Study, tallying and verifying the times for accuracy.
- Insure that the “Original Forms” file is up-to-date and copies are made from originals, not from copies.
- Assist the Staff Therapists with organization and copying of materials for presentations, in-services, etc.
- All other duties as assigned.
