We are seeking several dedicated RNs and LPNs to join our home health team. The ideal candidate must have a passion for patient care. We welcome any nurse: from new grad to ready to retire. Every other weekend and holiday rotation required.
Paid per visit.
$0.65/ mile between patients
$3.oo/ visit performance raise after 90 days.

Responsibilities:
- Provide direct patient care in the home setting
- Monitor patients' vital signs, review & administer medications, wound care, catheter care, education
- Perform assessments
- Collaborate with the interdisciplinary team to ensure optimal patient outcomes
- Educate patients and families on disease process and management
