Responsibilities
- Conduct thorough patient assessments to develop individualized care plans that address medical needs and personal preferences.
- Administer medications and fill pill boxes.
- Monitor vital signs, perform physical examinations, and evaluate patient progress during home visits.
- Provide education on disease management, medication administration, and health maintenance tailored to each patient's condition.
- Collaborate with physicians, case managers, and other healthcare professionals to coordinate comprehensive care delivery.
- Document all patient interactions.
Requirements
- Valid nursing license with current registration in the state of practice.
- Proven experience in home health care nursing or related settings such as hospice, long-term care, or hospital medicine.
- Knowledge of medical terminology, anatomy, physiology, and infection control practices.
- Experience working with individuals with disabilities or developmental delays is highly desirable.
- Strong communication skills coupled with a compassionate approach to patient-centered care.
