Location: Madera, California area
Compensation: $50 /hour plus benefits
Job Responsibilities:
- Provide high-quality Home Health care to a caseload of approximately 4 patients daily
- Conduct patient assessments , monitor vital signs, and develop care plans
- Assist with diagnostic evaluations and test result interpretations
- Collaborate with Medical Assistants, Nurse Practitioners, and administrative teams
- Offer triage services to determine the urgency of care
- Deliver specialized care for patients with memory-related conditions
- Maintain thorough and accurate patient records
Job Requirements:
- Active RN license in California
- Home Health or Hospice experience preferred
- Must have a reliable vehicle for travel
- Strong assessment, communication, and critical-thinking skills
- Ability to work collaboratively in a multidisciplinary team
