Job Description
This position is for a frontline certified community health worker who is a trusted member of and/or has a particularly close understanding of the community served. The position aims to address efforts focused on medical, behavioral, and social determinants of health factors that improve the well-being of individuals, families, and communities, with a specific focus on providing resources to pregnant community members.
Roles & Responsibilities under this position include, but are not limited to:
- Serve as a liaison between health organizations/systems, social services, and the community to facilitate access to services and improve the quality and cultural competence of service delivery
- Serve as an advocate and community connection point for community residents to facilitate access to services and community opportunities.
- Work with Medicaid Care Coordination teams to provide support to address needed services that impact health, such as transportation, food insecurity, housing, public benefit eligibility, education, and other related barriers
- Provide culturally appropriate health education and distribute targeted outreach materials promoting Medicaid Managed Care Plan services/value-added benefits.
- Advocate for clients when interacting with healthcare systems/partner services.
- Provide community outreach by attending and representing the organization at community meetings and events in settings such as nonprofit community-based organizations, faith-based ministries, public schools, and other community-based organizations while providing culturally appropriate information through the use of outreach materials and other avenues as appropriate.
- Engage community members/clients to participate in the assessment processes, including but not limited to screening for Medicaid, filling out report of pregnancy (ROPs), filling out health risk assessment (HRAs), and encouraging enrollment into Medicaid. As well as collaboratively develop a plan of care based on their individual needs, preferences, and objectives.
- Assist in identifying creative solutions to patient barriers and needs that may require innovative thinking to locate and refer to the appropriate resource(s).
- Work with members to develop healthcare goals and identify potential barriers to achieving healthcare goals.
- Utilize motivational interviewing techniques and active listening to collect and retain member information and incorporate responses as they are presented to complete the assessment -Identify when supplemental assessments are needed and conduct supplemental assessments.
- Evaluate the effectiveness of community events and outreach materials.
- Use professional knowledge to promote good judgment, decision-making and outreach skills.
- Compile reports on outreach activities as requested.

Minimum Educational Background
- High school education or equivalent experience
- Must have completed one of the approved programs below and passed the certification exam as a requirement of the position:
- Care Coordination Systems community health worker
- Central State community health worker training program
- Cincinnati Children’s Hospital Medical Center CHW certification course
- Cleveland State University School of Nursing community health worker program
- Cleveland Institute of Community Health
- Cuyahoga Community College community health worker program
- Health Care Access Now community health workers certification program

- Kettering College Community Health Worker Certificate Program
- Mercy College of Ohio community health worker program
- Ohio University community health worker training
- Pathways Community Hub (HCAN) Institute community health worker training program (Toledo OH)
- The OSU College of Nursing community health worker program
- Familiarity with technology (computer, phone), a valid driver’s license, reliable transportation, and working weekends or evenings is required for this position.
- Familiarity with working in health/ healthcare, basic understanding of community organizations/ social services resources, outreach, case management, or home health experience is preferred. Experience working with similar populations in related roles (s), such as social work or health education preferred.
- Must be able to draw in from lived expertise within challenges faced by the target community, such as medical, behavioral, and social determinants of health factors, and/or a deep understanding of the local community, its resources, and its social dynamics.
- Preferred: Bilingual (Spanish, Haitian Creole)
Pay: From $21.00 per hour
Expected hours: 40.0 per week
