Overview
Healthy Me Clinic is seeking an experienced Nurse Practitioner to provide primary care through house calls, assisted living facility rounding, and memory care across the DFW area.
This is a full time job. This opportunity offers competitive income, high retention bonus and flexible schedule.
This position is for a dependable, clinically confident provider who can manage a geriatric patient panel with limited day-to-day supervision.
Responsibilities
● Conduct scheduled house calls and assisted living facility rounds
● Perform new-patient and follow-up telehealth visits (optional)
● Evaluate and manage acute and chronic medical conditions
● Complete medication reconciliation and prescribe medications within the authorized scope
● Order and review laboratory tests, imaging, home health, therapy, and other clinical services
● Coordinate care with facility staff, families, specialists, and home health agencies
● Complete accurate EHR documentation promptly
Required Qualifications
● Active, unrestricted Texas Nurse Practitioner license
● Meaningful experience in geriatrics, primary care, assisted living, home-based care, or a comparable setting
● Ability to work independently within the delegated scope, clinic protocols, and applicable Texas requirements
● Willingness to travel to assigned DFW locations
● Fixed 3 weekday availability
Preferred Qualifications
● At least two years of Nurse Practitioner experience
● Prior assisted living, memory care, skilled nursing, or house-call experience
● Medicare credentialing
● Experience managing medically complex older adults
● Active DEA registration
● Familiarity with remote patient monitoring and chronic care management
