Community Wellness Coordinator
Healthy Community Alliance is a rural health network based in Gowanda dedicated to improving the health and quality of life for individuals and families across rural Western New York. We work collaboratively to strengthen community health, promote prevention, and support vulnerable populations. As part of our growing commitment to the wellbeing of older adults, we are seeking a passionate and organized Community Wellness Coordinator to join our team.
The Community Wellness Coordinator will play a key role in enhancing the health, independence, and social connection of older adults. This position works directly with older adults and our partners to conduct screenings, coordinate resources, and support healthy aging for older adults in the Springville, southern Erie County and northern Cattaraugus County area.
REQUIREMENTS/QUALIFICATIONS:
- Bachelor’s degree in health and human services or related field or Associates degree with 3 years related experience
- Experience in case management, service coordination and/or assisting clients in improving healthy behaviors, preference given to experience working with older adults
- Knowledge of community resources in and around the Springville, southern Erie County and northern Cattaraugus County area.
- Able to carry out carry out program workplan activities that address access to health and social care services for rural communities
- Able to establish and maintain relationships with older adult clients and service providers
- Able to manage time and schedules effectively
- Reliable transportation to and from worksites, meetings, or client homes across the service area of southern Erie County and northern Cattaraugus County.
- Strong communication and organizational skills
- Writing ability sufficient to provide adequate documentation in the client record, screening and referral forms and other service coordination forms, and reading ability to the level necessary to comprehend training materials and assist others to fill out forms.
Schedule: Potential ability to work some remote hours; Standard work week: Monday – Thursday (8:00-4:30); Friday (8:00-1:00) but may have non-traditional hours.

Salary: $22- $24 per hour
