We are seeking a compassionate and experienced Internal Medicine Physician to join our dynamic Post-Acute Division. This full-time position offers a rewarding opportunity to provide high-quality care to our skilled nursing facility patients in the St. Augustine and Jacksonville areas.
Internal Medicine Position Responsibilities:
Rounding:
- Monday-Friday rounding on 1-2 buildings per day, depending on census.
- NP support to assist with patient care.
Patient Care:
- Manage a patient load of 20-22 patients per day.
- Utilize Nursing Home EMR for efficient charting.
- Provide comprehensive care to skilled, short-term patients with 3 visits per week.
- Attend to 3-5 buildings as needed.
On-Call:
- Day call coverage from 8 AM to 6 PM for facilities rounded on that day.
