Assistant Vice President, Patient Access & Community Outreach - RN

A highly respected hospital and healthcare organization is seeking an experienced nursing leader to oversee admissions, intake, referral management, patient access, and community outreach operations across a complex care delivery network.


This executive leadership role will partner closely with senior leadership, referral sources, health systems, physicians, managed care organizations, and community providers to drive operational excellence, improve access to care, and support continued organizational growth. The position offers significant visibility, strategic influence, and oversight of multiple high-impact service areas.


Key Responsibilities

  • Lead hospital admissions, intake, referral management, and outreach operations.
  • Build and maintain relationships with hospitals, health systems, physician groups, skilled nursing facilities, and community referral sources.
  • Partner with executive leadership to support strategic growth initiatives and expand market presence.
  • Oversee departmental performance, staffing, budgeting, quality improvement, and operational metrics.
  • Collaborate with managed care organizations and payers to facilitate patient access and service delivery.
  • Ensure compliance with regulatory requirements and quality standards.
  • Represent the organization at community and professional events throughout the region.


Ideal Background

  • Active NYS RN license required.
  • BSN required; MSN preferred.
  • 5 years of progressive healthcare leadership experience.
  • Strong background in home health, care transitions, patient access, admissions, referral management, discharge planning, or community outreach.
  • Experience managing multidisciplinary teams and operational performance.
  • Demonstrated ability to build relationships with referral sources and drive organizational growth.
