The Integrated Care Center Clinic is a federally qualified health centers that provide primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need including those who are suffering from homelessness, mental illness, substance use, mental illness, and chronic medical conditions. We provide team based care in an integrated care setting. HealthRIGHT 360’s Mobile and Street Medicine program delivers comprehensive, high-quality medical care and social services to individuals experiencing homelessness in San Francisco. Rooted in the mission to "go to the people", the program offers services directly to patients—on the street, in encampments, or via a fully equipped mobile medical van. The program operates as an extension of our Integrated Care Center (ICC), expanding the reach of our services beyond the clinic walls. This new and rapidly growing initiative is committed to providing low-barrier, trauma-informed, and person-centered care in the environments where people live. Our multidisciplinary team prioritizes trust, continuity, and access while delivering compassionate, dignified, and high-quality services to some of the city’s most underserved populations.

As a key member of a multidisciplinary team, the Medical Assistant (MA) supports licensed providers and nursing staff in delivering high-quality, patient-centered care in non-traditional, community-based settings. This hybrid role involves both fieldwork and office-based responsibilities. In the field, the MA assists with outreach efforts and provides direct patient support, including registering patients, scheduling appointments, taking vital signs, performing point-of-care tests and phlebotomy, administering injections, assisting with clinical procedures such as wound care, and supporting limited case management activities. In the office, the MA is responsible for maintaining and preparing medical supplies for field use, managing inventory, completing prior authorizations, requesting medical records, and performing other administrative tasks to support both the care team and patient needs.

Key Responsibilities

Medical Responsibilities

  • Perform patient intake, including obtaining vital signs, listing chief complaints, allergies, and immunization histories, and completing appropriate chart forms.
  • Assist clinicians in the examination process in the field, including during mobile clinic and street-based (backpack medicine) encounters.
  • Perform common diagnostic tests, including vision screening, spirometry, EKG, rapid strep, urinalysis, and urine pregnancy tests.
  • Conduct phlebotomy, administer injections and TB tests, and provide nebulizer/breathing treatments.
  • Assist with wound care and other hands-on procedures as needed in field conditions.
  • Participate in the Enhanced Care Management (ECM) program by providing care coordination and basic case management services to patients experiencing homelessness.
  • In the office, manage clinical tasks such as completing prior authorizations, responding to inbox messages and tasks, completing medical record requests, coordinating with the referrals team, verifying insurance authorizations, scheduling appointments, and performing patient registration.
  • Act as a patient advocate, helping individuals navigate complex medical and social systems.

Preparation Responsibilities

  • Clean, stock, and prepare mobile medical van exam rooms and ensure all supplies are available for field deployment.
  • Work with administrative support to maintain and organize supply inventories for both the mobile medical van, Street Medicine backpacks, and office-based care settings.
  • Assist in packing and maintaining portable medical kits/backpacks and field equipment used in Street Medicine outreach.

Coordination Responsibilities

  • Receive and return calls from patients, pharmacies, external providers and community organizations in a timely and compassionate manner.
  • Assist in scheduling specialty appointments, follow-ups, and consultations.
  • Monitor the quality assurance of the mobile medical van and overall Street Medicine operations, reporting concerns and contributing to improvements.
  • Support efficient patient flow and coordination of care during mobile clinic days and outreach efforts.
  • Collaborate with community partners and service providers to support continuity of care and referrals.

Outreach & Field-Based Care

  • Participate in regular street outreach with a multidisciplinary team, including navigating encampments, shelters, parks, and sidewalks to engage patients where they live.
  • Support patient engagement by building trust over time through trauma-informed, culturally humble communication.
  • Apply harm reduction principles when engaging with patients, including nonjudgmental support around substance use, safer use practices, and connecting individuals to appropriate resources without requiring abstinence.
  • Documents care promptly according to charting protocols and clinic policies in the electronic medical record (EMR).
  • Monitor and promote safety in field environments in collaboration with clinical and outreach team members.

Emergency & Crisis Response

  • Identify and respond to emergent medical or behavioral health needs encountered in the field.
  • Assist with basic triage and support de-escalation, when necessary, while ensuring timely referral to emergency services.
  • Carry and assist with the administration of emergency supplies such as naloxone, glucose, or epinephrine when indicated.

Qualifications

Education, Experience and Credentials

  • High school diploma or equivalent accreditation required.
  • Completion of a Certified Medical Assistant (MA) program from an accredited institution preferred.
  • 2 years’ experience working as a Medical Assistant (preferably in community health, public health, or outreach-based settings).
  • First Aid and CPR certification.
  • Valid driver’s license and ability to travel throughout San Francisco to outreach locations.
  • Preferably certified or documented experience in phlebotomy.

Background Check And Other Requirements

  • Qualified candidates with arrest and conviction records will be considered for employment.
  • Must be capable of completing a background check and live scan.
  • Must complete background check and Livescan.
  • Covid-19 vaccine and booster is required for this position. Exemptions are not accepted.
