Licensed Practical NursePosition Summary:The Licensed Practical Nurse (LPN) provides community-based nursing support to a Family Nurse Practitioner (FNP) serving adults with mental health needs. This role focuses on delivering compassionate, high-quality care in home and in the community. The LPN assists with care coordination, medication support, health education, and monitoring of clients with behavioral health conditions, helping promote stability, wellness, and improved quality of life.Job Responsibilities: Maintain total confidentiality and adhere to HIPAA law, CARF accreditation standards and Maryland State RegulationsAssist the Family Nurse Practitioner with client assessments, follow-up visits, and care plan implementationPerform basic nursing tasks including vital signs, symptom monitoring, wound care (if applicable), and medication administration under supervision and within scopeConduct community-based visits to monitor client health status, medication compliance, and psychosocial needsUsing electronic medical records (EMR), input data and following through seeing services to their completionSupport mental health interventions by observing, documenting, and reporting behavioral changes and risk concerns to the FNP or clinical teamProvide medication education and reinforce treatment plans developed by the FNPAssist the FNP with collecting data, updating treatment plans, and completing required forms or reportsAdminister prescribed medications, including injections under the supervision of the prescriber or register nurseCollaborate with the FNP, therapists, case managers, social workers, and external providers.Take part in all agency training programs as requestedQualifications, Educations and Experience: Must possess an active and unrestricted license as a Practical NurseMinimum 2 year of nursing experience; behavioral health, community health, or home care experience preferredValid driver’s license with a safe driving recordStrong communication, documentation, and problem-solving skillsDemonstrated ability to work collaboratively in a teamwork environmentCurrent CPR and First Aid certification preferredWhy work for HealthPort?HealthPort is a Certified Community Behavioral Health Center that offers outpatient therapy, substance abuse, and primary care services.
