Licensed Practical Nurse (LPN)- Full‑Time
Location: Salisbury, MD and surrounding counties
Salary Range: $50,000–$70,000 annually
Schedule: Four‑day, 36-hour work week — Monday through Thursday
Position Summary
The Licensed Practical Nurse (LPN) plays a vital role in HealthPort’s integrated behavioral health model, working directly with a Family Nurse Practitioner (FNP) to support adults receiving community‑based health services. The LPN provides in‑home and community nursing care, health monitoring, medication support, and care coordination to help clients achieve stability, wellness, and an improved quality of life.
Key Responsibilities
- Uphold strict confidentiality in accordance with HIPAA, CARF standards, and Maryland regulations.
- Support the FNP with assessments, follow‑up visits, and implementation of individualized care plans.
- Perform nursing tasks such as vital signs, symptom monitoring, wound care (if applicable), and medication administration within scope and supervision.
- Conduct community and home visits to assess health status, medication adherence, and psychosocial needs.
- Document services in the electronic medical record (EMR) and ensure timely follow‑through on required actions.
- Observe, document, and report behavioral changes or risk concerns to the FNP and clinical team.
- Educate clients on medications and reinforce treatment plans developed by the FNP.
- Assist with data collection, treatment plan updates, and completion of required forms and documentation.
- Administer prescribed medications, including injections, under appropriate supervision.
- Collaborate with multidisciplinary team members including FNPs, therapists, case managers, and external providers.
- Participate in all required agency training programs.
Qualifications & Experience
- Active, unrestricted Practical Nurse (LPN) license.
- Minimum of 2 years of nursing experience; behavioral health, community health, or home‑based care preferred.
- Valid driver’s license and clean driving record.
- Strong communication skills and proficiency in documentation and problem‑solving.
- Demonstrated ability to work collaboratively as part of a multidisciplinary team.
- Current CPR and First Aid certification preferred.
