HealthOp is seeking an experienced Director of Value-Based Care to lead strategic initiatives that improve patient outcomes, strengthen care coordination, and optimize performance across value-based contracts.
Based in Scottsdale, Arizona, this leadership role is ideal for a healthcare executive with deep expertise in value-based care, population health, risk-based reimbursement, and quality improvement. You'll collaborate with executive leadership, clinical teams, analytics, and operational partners to drive innovation, operational excellence, and measurable results across a growing physician network.
What You'll Do:
✅ Lead value-based care strategy across risk-bearing contracts and programs
✅ Oversee Risk Stratification, Quality Gap Closure, and Population Health teams
✅ Improve quality, financial, and operational performance through data-driven initiatives
✅ Partner with executive leadership to advance patient-centered care
✅ Build strong relationships with payers, providers, and community partners
✅ Mentor and develop high-performing multidisciplinary teams
What We're Looking For:
✔️ Bachelor's degree required (Master's preferred)
✔️ 7–10 years of healthcare leadership experience in Value-Based Care, Population Health, ACOs, or risk-based reimbursement
✔️ Strong knowledge of HEDIS, HCC coding, risk adjustment, healthcare analytics, and quality improvement
✔️ Proven leadership in physician practice, medical group, or multi-site healthcare environments
