Director of Case Management

This is a full-time, on-site role based in Orlando, FL, for the Director of Case Management. The Director of Case Management will oversee daily operations related to discharge planning, utilization management, medical case management, and compliance with Medicare regulations. Responsibilities also include leading a team to ensure high-quality, patient-centered care while optimizing resource utilization and improving outcomes. This role involves collaboration across multiple departments to drive effective case management practices.

• Expertise in Discharge Planning and Utilization Management
• Proficiency in Medical Case Management and overall Case Management practices
• Strong understanding of Medicare regulations and compliance requirements
• Exceptional leadership, organizational, and communication skills
• Bachelor's degree in Nursing or a related healthcare field; Master’s degree preferred
• Current RN license or a relevant healthcare certification
• Experience in a hospital or acute care setting is highly desirable
