Initial Scope of Work Summary: The Director of the HealthKeeperz Tailored Care Management (TCM) program is entrusted with the crucial task of establishing, developing, and supporting the TCM program at HealthKeeperz. Working closely with the Human Resources and Quality Improvement/Compliance departments, the director will spearhead the creation of the TCM program, ensuring its alignment with HealthKeeperz's mission and vision.
Initial Key Expectations:
- Building the TCM Program: The director will collaborate with relevant departments to design and implement the program, outlining its structure, processes, and objectives. This includes identifying and acquiring qualified supervisors, case managers, and support staff to form a dedicated team of caregivers. At times the director will be expected to assist supervisors or case managers with direct beneficiary care, which may include carrying a caseload.
- Program Staffing and Leadership: The director will be responsible for recruiting, hiring, and training the TCM program's staff, ensuring they possess the necessary skills and qualifications to deliver exceptional care. Providing strong and compassionate leadership, the director will inspire and guide the team to deliver person-centered care and support.
- Program Growth: The director will play a pivotal role in the growth and expansion of the TCM program. This involves identifying opportunities for expansion, establishing partnerships with relevant stakeholders, and implementing strategies to increase the program's reach and impact.
Ongoing Scope of Work Summary: Once the program is operational and adequately staffed, the director is not expected to be involved directly with beneficiary care, but rather the Director of Tailored Care Management at HealthKeeperz would be responsible for overseeing and leading the program in all HealthKeeperz’s North Carolina locations. This position requires in-depth knowledge of the NC Tailored Care Management guidelines, regulations, and best practices. The Director will ensure the effective implementation and execution of the program, maintain quality standards, and provide leadership and guidance to the Tailored Care Management team. They will collaborate with stakeholders, manage staff, and promote the delivery of person-centered care to individuals enrolled in the Behavioral Health Intellectual/Developmental Disabilities (BH I/DD) Tailored Plan.
Ongoing Key Expectations:
- Program Management and Leadership: Oversee the overall operation and management of the Tailored Care Management program.
- Develop and implement program policies, procedures, and protocols in alignment with NC Tailored Care Management guidelines and regulatory requirements.
- Ensure compliance with all relevant regulations, standards, and quality measures.
- Provide strategic direction and leadership to the Tailored Care Management team.
- Staff Management and Development: Recruit, hire, train, and supervise qualified staff for the Tailored Care Management program.
- Conduct performance evaluations, provide regular feedback, and identify opportunities for professional development and growth.
- Foster a supportive and collaborative work environment that promotes teamwork and individual accountability.
- Care Coordination and Quality Assurance: Oversee the development and implementation of individualized care plans that address the needs of enrolled individuals across physical health, behavioral health, I/DD, pharmacy, LTSS, and other relevant areas.
- Ensure effective coordination of services, including appointments, referrals, and access to community-based resources.
- Monitor and evaluate the quality and outcomes of care management services, identifying areas for improvement and implementing quality assurance measures.
- Stakeholder Collaboration: Collaborate with Medicaid-managed care organizations, healthcare providers, community organizations, and other stakeholders to foster effective partnerships and promote coordinated care for enrolled individuals.
- Represent the Tailored Care Management program in meetings, committees, and external forums.
- Program Evaluation and Reporting: Collect, analyze, and report program data and outcomes to measure the effectiveness and impact of the Tailored Care Management program.
- Prepare reports, presentations, and updates for internal and external stakeholders, as required.
- Budget Management: Manage the budget for the Tailored Care Management program, ensuring efficient allocation of resources and adherence to financial guidelines.

Education/Certifications
- A master’s-level fully Licensed Clinical Social Worker (LCSW), fully Licensed Clinical Mental Health Counselor (LCMHC), fully Licensed Psychological Associate (LPA), fully Licensed Marriage and Family Therapist (LMFT), or licensure as an RN;
- AND Three years of experience providing care management, case management, or care coordination to the population being served.
