- Document assessments, care plans, and progress; communicate with the care team to adjust plans as needed
- Support continuity of care through coordination across services and settings
Requirements
- Active RN license
- Experience in care management or home-based patient care is typical for this role (as described in the responsibilities)
- Ability to work independently while collaborating with an interdisciplinary team
- Strong communication and documentation skills
