Responsibilities
- Conduct occupational therapy evaluations and develop individualized treatment plans for patients requiring OT services.
- Implement evidence-based OT interventions aimed at activities of daily living, mobility, and functional independence; monitor progress toward goals.
- Document patient status and progress in HCHB and other required systems; maintain timely, accurate notes in alignment with policy.
- Collaborate with the interdisciplinary team (including physicians, nurses, and families) to support care planning, discharge planning, and home program instructions.
- Ensure patient safety and infection control; adhere to AHA BLS requirements; provide patient and caregiver education on prescribed home programs.
- Maintain compliance with regulatory requirements and facility policies; no on-call and no weekend shifts.
Requirements
- Occupational Therapy experience: minimum 2 years
- Recent specialty experience: minimum 3 months
- Knowledge of HCHB and at least 1 year of home health experience as an OT
- AHA BLS certification required
- Must be 21 years of age or older
- Ability to work a day shift on a travel assignment and function effectively within a Critical Access Hospital environment
