- Review resident medical records to determine appropriate level of care and support utilization management decisions
- Coordinate with the interdisciplinary care team to develop and implement care plans aligned with patient needs and payer requirements
- Conduct chart reviews to identify gaps, support discharge planning, and monitor for timely care transitions
- Obtain prior authorizations and communicate with payers and providers as needed
- Document all utilization management activities in the charting system in compliance with HIPAA and facility policies
- Active RN license in Pennsylvania (or eligibility for licensure in PA)
- Experience in utilization management, care coordination, or related field preferred
- Strong chart review, documentation, and communication skills
- Ability to work independently within a skilled nursing/long-term care setting
- Availability to work the scheduled shift and 40 hours per week
- Guaranteed hours for the assignment
- Weekly pay range provided
- Contract duration of 4 weeks
- Start date of June 1, 2026
