Responsibilities

• Implement and modify therapy plans of care under supervision of the licensed PT, delivering prescribed therapeutic exercises, gait training, balance activities, and functional mobility tasks
• Conduct individual therapy sessions, monitor patient responses, document progress, and adjust interventions as appropriate
• Assist with transfer training, ADLs, and device training to promote independence and safety for residents
• Maintain accurate patient records, including progress notes and outcome measures, and communicate changes to the supervising PT and care team
• Ensure safety, infection control, and equipment maintenance; collaborate with the interdisciplinary team to support discharge planning and continuity of care
• When applicable, document and reference Rehab Optima / NetHealth data entries and contribute to rehabilitation documentation as part of the patient care plan
  
  

Requirements

• Active California Physical Therapy Assistant license
• SNF experience preferred; NetHealth (Rehab Optima) knowledge would be a plus
• Ability to work day shift in a travel assignment within a skilled nursing facility setting
• Strong communication and teamwork skills; ability to follow directions from supervising PT and align with facility policies
• Comfort creating and teaching home exercise programs and caregiver education as part of step-down care
