Responsibilities

• Conduct comprehensive occupational therapy evaluations for residents within an acute care–informed SNF setting
• Develop and implement individualized treatment plans focused on activities of daily living and functional independence
• Deliver direct OT interventions using evidence-based approaches; adjust plans based on progress
• Document assessments, treatment plans, progress notes, and discharge summaries in a timely, compliant manner
• Collaborate with the interdisciplinary team (nursing, PT, SLP, social work) to coordinate care and planning for safe transitions
• Educate residents, families, and caregivers on home exercise programs, safety strategies, and adaptive equipment as appropriate
  
  

Requirements

• Active Occupational Therapist license in Washington state
• Master’s degree in Occupational Therapy from an accredited program
• Minimum of 1 year of clinical experience in acute care or skilled nursing settings preferred
• Ability to work in a skilled nursing facility environment; willing to participate in travel assignment as scheduled
• Current CPR/BLS certification or ability to obtain
