Responsibilities

• Administer medications and treatments as prescribed, following facility policies and state regulations
• Conduct resident assessments, monitor vital signs, and identify changes that require intervention
• Document care and communication notes accurately in the electronic health record
• Provide direct nursing care during the night shift, including activities of daily living support and safety monitoring
• Collaborate with the interdisciplinary team to update and implement resident care plans
• Report concerns to supervising clinicians promptly and respond to emergency situations as needed
  
  

Requirements

• Active Licensed Practical Nurse (LPN) license in New Hampshire
• Graduation from an approved practical nursing program
• Ability to work night shift (11:00 PM–7:30 AM) in a skilled nursing setting
• Prior long-term care experience preferred
• CPR/BLS certification preferred or as required by policy
• Eligible to work in the United States and able to perform a local assignment in Keene, NH
