Responsibilities

• Provide routine nursing care to residents, including monitoring condition and delivering treatments per plan of care
• Administer medications and validations, observe for adverse effects, and document responses accurately
• Perform head-to-toe assessments, document findings, and report changes to the supervising RN
• Assist with activities of daily living, mobility, wound care, and routine hygiene for residents
• Collaborate with the interdisciplinary team to implement and update individualized care plans
• Maintain infection control practices, safe medication administration, and resident safety; respond to emergencies and escalate as needed
  
  

Requirements

• Active Licensed Practical Nurse (LPN) license in the state of practice
• Graduation from an approved practical nursing program
• Current Basic Life Support (BLS) certification or the ability to obtain quickly
• Prior experience in long-term care or skilled nursing facilities preferred
• Ability to work 12-hour day shifts with a Fri–Thu facility work week
• Strong communication, documentation, and teamwork skills; proficient with standard clinical software and charting
