Responsibilities

• Administer prescribed medications and document administration accurately under the supervision of the nursing team
• Perform focused assessments, monitor resident status, and report changes promptly to the charge nurse
• Provide direct resident care, including activities of daily living, mobility assistance, and routine hygiene
• Assist with wound care, vital signs, and routine clinical procedures within scope of practice
• Maintain accurate, confidential documentation and ensure a safe, clean, and comfortable environment
• Collaborate with interdisciplinary team to implement individualized care plans and respond to resident needs and call lights efficiently
  
  

Requirements

• Active Licensed Practical Nurse license in Vermont (or eligibility for VT licensure)
• Graduation from an approved practical nursing program (diploma or degree)
• Current CPR/BLS certification
• 1 year of recent licensed practical nurse experience in long-term care or skilled nursing preferred
• Strong communication, critical thinking, and teamwork skills
• Ability to work evenings within a 40-hour weekly schedule in a long-term care setting
