Responsibilities

• Obtain medication histories from patients and gather relevant home medication information
• Reconcile home medications with those prescribed during hospitalization
• At discharge, identify medication discrepancies and communicate findings to prescribers for reconciliation
• Perform standard, structured medication reconciliation tasks as part of the care process
• Document and communicate medication-related issues to the care team to support safe transitions of care
  
  

Requirements

• Experience with medication histories and reconciliation or pharmacy workflow preferred
• Ability to communicate clearly with patients and prescribers
• Strong attention to detail and accuracy in documentation
• Comfortable working in a hospital setting and collaborating with a multidisciplinary team
