We are seeking a Nurse Specialist to join our care team! This role is about more than clinical skills—it’s about building trust, providing education, and ensuring our patients feel supported every step of the way.
In this role, you will:
Serve as the first point of contact for patients and families with health questions or concerns.
Support patients with care coordination, triage, and medication management.
Provide education and counseling to empower patients in managing their health.
Collaborate with providers, community health workers, and other team members to connect patients with the care and resources they need.
Advocate for patients facing barriers—whether that’s transportation, prescriptions, or navigating the healthcare system.
Qualifications:
Active RN license in Kentucky and BLS certification required.
Nursing degree (Diploma, ADN, BSN, or MSN) with relevant nursing/care coordination experience.
A passion for equity, compassion, and community health.
