About the Role
The Nurse Specialist – Triage Lead is a key member of our care team, providing high-quality telephone triage, patient education, and care coordination. In addition to clinical responsibilities, you’ll lead triage operations, train staff, audit workflows, and ensure safe, efficient, patient-centered care. This role is perfect for a nurse who thrives in leadership and collaboration.
What You’ll Do
- Clinical & Triage:
- Serve as a trusted resource for patients and families with clinical questions
- Triage acute medical issues using established protocols
- Communicate test results and care instructions per provider direction
- Provide patient education and self-management support
- Document all interactions accurately in the EMR
- Leadership & Team Support:
- Oversee triage workflows and daily huddles
- Train and mentor triage nurses
- Conduct chart audits and ensure compliance with protocols
- Assist with scheduling, evaluations, and team development
- Care Coordination:
- Track patients after hospital or ER visits
- Educate patients on care plans and assist with goal setting
- Connect patients to internal and external resources for social or financial needs
What We’re Looking For
- Current RN license in Kentucky
- CPR certification
- Education & Experience:
- Nursing Diploma 3 years experience OR
- ADN 5 years experience OR
- BSN 1 year experience OR
- MSN 1 year experience
- Strong communication, leadership, and organizational skills
- Passion for patient-centered care and team collaboration
