We are seeking an experienced and highly capable clinical professional to support our innovative skilled nursing facility (SNF) program. This role is ideal for a Certified Nursing Assistant (CNA) or Medication Technician (Med Tech) who is also a trained phlebotomist and has a strong background in long-term care.
In this position, you will operate and manage a diagnostic cart within a skilled nursing facility, performing a variety of clinical tasks and facilitating telehealth visits with a remote physician. You will serve as a critical link between the patient, on-site facility staff, and the telehealth provider to ensure seamless, high-quality care delivery.

Responsibilities
Diagnostic Support
- Operate and manage a diagnostic cart used to perform point-of-care and advanced diagnostic testing within the skilled nursing facility
- Perform phlebotomy and specimen collection with accuracy and adherence to protocols
- Assist in administering diagnostic tests and ensuring proper handling of equipment and results
- Maintain and troubleshoot equipment as needed to ensure consistent functionality
- Provide direct patient support during diagnostic procedures with professionalism and compassion
- Collaborate closely with skilled nursing facility staff to coordinate care and testing
- Ensure patient comfort, safety, and understanding throughout all interactions
- Maintain strict adherence to infection control and safety protocols
- Coordinate and support telemedicine visits between patients, facility staff, and the remote physician
- Prepare patients and clinical information for virtual consultations
- Communicate real-time findings and assist the physician in executing care plans
Qualifications
- Must be a Certified Nursing Assistant (CNA) or Medication Technician (Med Tech)
- Phlebotomy certification REQUIRED
- Required: Prior experience working in a skilled nursing facility (SNF) or long-term care setting
- Candidates without SNF/long-term care experience will not be considered
- Demonstrated experience performing phlebotomy in a clinical setting
- Strong understanding of geriatric patient care
- Comfortable working with technology, diagnostic equipment, and telehealth platforms
- Excellent communication and teamwork skills
- Ability to work independently while coordinating with a remote clinical team
