PRN Position Details:
- Provide high-quality physical therapy services to residents, including assessments, treatment planning, and therapy sessions.
- Collaborate with a multidisciplinary team to develop and implement individualized care plans.
- Monitor and document residents’ progress, adjusting treatment plans as needed.
- Educate residents and their families on therapeutic exercises and injury prevention.
- Ensure compliance with all regulatory and safety standards.
- Maintain accurate and timely documentation of services provided.
Skills and Qualifications:
- Valid state license as a Physical Therapist.
- Proven experience in geriatrics or long-term care settings preferred.
- Strong interpersonal and communication skills.
- Ability to work independently or as part of team.
- Flexibility to work some weekday hours to accommodate resident needs.

Requirements
Applicants must have an active Indiana Physical Therapist license or must have the necessary qualifications to earn Indiana license. These qualifications include graduation from a Commission on Accreditation in Physical Therapy education program, subsequent passing of a state-administered national exam, and fulfilling the state’s requirements for licensure. Applicants must be legally eligible to work in the United States. Proof of eligibility will be required upon offer of employment.
