Duties:

• Delivers skilled nursing care through in-home visits to support healing and manage chronic conditions.
• Performs initial OASIS assessments and follow-up visits as scheduled or clinically indicated.
• Educates patients and families on disease progression and effective care management.
• Manages a daily caseload of 3–5 in-home visits, based on patient needs and scheduling.
• Efficiently applies HCHB EMR experience to ensure accurate and timely documentation.

Required:
• At least 1 year of recent, full-time geriatric home health RN experience (excluding PRN)
• Proficient with OASIS
• HCHB software charting experience
