Monday–Friday, 8:00am–5:00pm
Daily Responsibilities
- Lead and support the nursing team to ensure safe, coordinated, high?quality care for older adults in the PACE program
- Oversee nursing services across the PACE center and in participants’ homes, stepping in for hands?on care when needed
- Serve as the primary clinical expert for complex medical situations and interdisciplinary care planning
- Supervise, train, and develop nursing staff while ensuring compliance with PACE regulations and safety standards
- Provide and oversee home?based visits within the Springfield area
- Participate in on?call rotation as needed (details discussed during interview)
- 5 years of progressively responsible nursing experience in PACE, acute care, ambulatory, long?term care, home health, or similar settings
- 1 year of direct clinical experience with frail, elderly, or medically complex older adults
- Prior nursing leadership or supervisory experience (charge nurse, supervisor, manager, etc.)
- Current, unrestricted Oregon RN license
- Valid CPR certification (BLS or ACLS acceptable)
- Proficiency with electronic medical records
- Panel interview with 2–3 PACE leaders
- Conducted via Microsoft Teams
