Daily Responsibilities For RN Case Manager
- Manage high-risk cases supporting children, adults, and seniors with complex medical, behavioral, and social needs
- Develop and implement individualized care plans such as conducting assessments, care coordination, and engaging members using evidence-based approaches
- Conduct field based member visits
- Active/Unencumbered RN License in the state of California
- Minimum 3 years of case management experience in a healthcare delivery setting
- Must have a valid California Drivers License with valid automobile insurance
- Advanced computer proficiency
- Managed care experience!
- Bachelor’s degree in nursing
- Bilingual (English/Spanish)
- Very low healthcare premiums

Salary : $91,000 - $121,000
