Patient Care Manager / Registered Nurse (Mon-Fri, 8a-5p)

Healthcare Support is seeking a Patient Care Manager / Registered Nurse for a leading home health organization in Temple, TX.

This is an excellent opportunity for an RN with home health experience to oversee a clinical team, ensure quality patient care, and support daily operations in a primarily office-based role

Pay Details: $90,000 - $95,000 a year (based on experience)

Schedule: Monday-Friday 8am-5pm

Daily Responsibilities for Patient Care Manager / Registered Nurse:

• Oversee a patient census of approximately 150-175 patients and manage a team of 10-20 clinicians and support staff while ensuring coordinated, high-quality patient care
• Lead, coach, and support clinical staff through team meetings, training, evaluations, one-on-one development, and performance management
• Manage daily clinical operations including scheduling, order approvals, patient and family concerns, and ensuring compliance with quality standards and regulatory requirements
• Serve as a clinical resource for the team and provide occasional field support, staff training, or patient care coverage when needed
  
  

Required Qualifications For Patient Care Manager / Registered Nurse

• Active RN license in the practicing state
• Current CPR certification
• 1-2 years of recent leadership experience
• Prior home health experience required
• Experience with OASIS documentation
• Knowledge of Medicare Conditions of Participation and federal, state, and local regulatory requirements
• Valid driver's license and insured, reliable transportation
