Mobile Nurse Practitioner

HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with one of the nation’s largest healthcare organizations here in the NY area.

Daily Responsibilities for Mobile Nurse Practitioner:

• Provide care at assigned nursing homes and clinical sites, seeing patients regularly.
• Supervise and support other clinical staff at your location.
• Conduct annual wellness assessments for site participants, considering their complex health needs.
• Offer urgent care services, ensuring timely access and managing acute conditions.
• Follow up with participants within 5 days of hospital/ED discharge, including medication reconciliation and coordinating care with the team.
• Work Monday to Friday, 8am – 5pm.
  
  
  

Required Qualifications for Mobile Nurse Practitioner:

• Licensed to practice as a Nurse Practitioner in NY
• DEA Certification
• AANP or ANCC Board Certification
• 1 years of experience working with Geriatric population
• 2 years of experience in clinical nursing practice in-home, care management, or related field

Pay Details: Up to $180k – 190k Annual Bonus
