As a Home Health Physical Therapist, you’ll have the opportunity to provide personalized, one-on-one care to patients in their homes.
Daily Responsibilities
- Provide skilled in-home physical therapy care, evaluating patients’ mobility, safety, strength, balance, and functional abilities, then creating personalized treatment plans that help them recover independence after illness, injury, or surgery.
- Work independently in the field while collaborating with the care team, delivering one-on-one therapy in patients’ homes, educating patients and families on exercises and safety techniques, and monitoring progress to ensure the best possible outcomes.
- Typically seeing 3-6 patients/day depending on the type of visit.
- Minimum nine months’ experience as a physical therapist
- Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
- Currently licensed to practice as a physical therapist in Colorado
- Must be licensed driver who can travel to all business locations
