HealthCare Support is actively seeking a full-time Senior UAS Assessor to support members in Buffalo, NY. Schedule:
- Monday – Friday, 8:30 AM – 5:00 PM
- Work-life balance: No weekends, no holidays, no night shifts
Compensation Notes:
- $90,000 – $100,000 a year (based on experience)
- Up to 8% Annual Bonus
- Mileage & toll reimbursement
Daily Responsibilities for the Senior UAS Assessor:
- Assess and coordinate medical and support services across the continuum of care for complex, high-acuity populations to promote quality, cost-effective outcomes
- Develop and manage personalized long-term care plans while educating members and families on available services and benefits
Required Qualifications for the Senior UAS Assessor:
- Active and clear NY State RN license
- Must have Home Health or UAS experience as an RN
