HealthCare Support is actively seeking a RN Field Case Manager to fill an opening in Lillington, North Carolina.
Daily Responsibilities for RN Field Case Manager:
- Ensure accurate and complete documentation.
- Communicate and collaborate with the interdisciplinary care team
- Assist the provider/team with various care coordination activities
- Monday to Friday 8AM - 5PM EST
- Active and unrestricted RN license in North Carolina
- Associate degree or higher in Nursing (RN)
- Certified in Basic Life Support
- 1 year of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
