Field Nurse Practitioner

Field Nurse Practitioner – up to $150,000/year bonus! 

HealthCare Support is seeking a Field Nurse Practitioner (NP) to join a rapidly growing healthcare provider serving patients in Casa Grande, AZ.

This is a unique opportunity to deliver high-quality, patient-centered care directly in the home—focusing on Chronic Kidney Disease and primary care needs. Schedule: Monday – Friday, 8:00 AM – 5:00 PM

Compensation Notes: $140,000 – $150,000 annually 10% annual bonus
• Mileage reimbursement provided

Daily Responsibilities for the Field Nurse Practitioner:

• Conduct comprehensive in-home patient visits, including history and physical exams
• Develop and implement individualized treatment plans
• Order and interpret diagnostic tests and lab work
• Prescribe medications as appropriate
• Provide ongoing patient education and chronic disease management
• Complete accurate and timely documentation of services
• Collaborate with interdisciplinary care teams and participate in routine meetings
• Utilize telehealth to support patient care when appropriate

Required Qualifications for the Field Nurse Practitioner:

• Active Nurse Practitioner (NP) license in Arizona
• 2 years of experience as a practicing NP
