HealthCare Support is actively seeking a to fill Field Care Manager Bilingual an opening in Cape Coral , Florida.
Daily Responsibilities for Field Care Manager Bilingual:
- Visit Medicaid members in their homes, Assisted Living Facilities, Long Term Care Facilities, and other care settings
- 75-90% local travel required
- Assess and evaluate member's needs and requirements to establish member-specific care plans
- Ensure members receive services in the least restrictive setting to achieve and/or maintain optimal well-being
- Monday to Friday 8AM - 4:30PM
- Must live in Charlotte or Lee counties in Florida
- Bilingual in English and Spanish
- Bachelor's degree in health and human services field
- 2 or more years of related experience in case management: assessments/care planning etc.
- Prior experience with Medicare & Medicaid recipients
- Experience with electronic case note documentation and documenting in multiple computer applications/systems
- Experience with health promotion, coaching, and wellness
- Knowledge of community health and social service agencies and additional community resources
