Home Health RN - Nurse Manager

The Home Health RN - Nurse Manager plays a critical role in overseeing patient care while managing and guiding a team of nursing professionals within a home health setting. This position ensures the development and implementation of effective care plans, maintains quality and regulatory standards, and fosters interdisciplinary collaboration to optimize patient outcomes.

Responsibilities

• Oversee patient care delivery and ensure adherence to care plans
• Manage and lead the home health nursing team
• Conduct staff training and professional development sessions
• Develop and update individualized care plans
• Implement quality assurance measures
• Ensure compliance with healthcare regulations and policies
• Allocate resources effectively to meet patient and team needs
• Coordinate with interdisciplinary healthcare professionals
• Evaluate staff performance to maintain high standards

Required Qualifications

• Minimum 2 years of experience in home health nursing
• Associate or Bachelor's Degree in Nursing (BSN)
• Current Registered Nurse (RN) License
