RN Case Manager – Hospice & Palliative Care
We are seeking an experienced RN Case Manager to join a highly respected hospice and palliative care organization serving patients throughout the Fresno area.
This role offers the opportunity to provide compassionate, patient-centered care to individuals and families navigating serious illness and end-of-life needs. As a key member of an interdisciplinary care team, you will coordinate care plans, manage patient needs across the continuum of care, and serve as a trusted clinical resource for patients, families, and caregivers.
Key Responsibilities
- Manage a caseload of hospice and palliative care patients in a community-based setting.
- Conduct comprehensive nursing assessments and develop individualized plans of care.
- Coordinate services with physicians, social workers, chaplains, home health aides, and other interdisciplinary team members.
- Monitor patient status, identify changes in condition, and implement appropriate interventions.
- Provide education and support to patients, families, and caregivers regarding disease progression, symptom management, and available resources.
- Ensure accurate clinical documentation and compliance with regulatory standards.
- Participate in care conferences and collaborate on patient-centered treatment plans.
- Support patients and families with compassion, dignity, and respect throughout the care journey.
Qualifications
- Active California Registered Nurse (RN) license.
- Current CPR certification.
- Minimum of one year of nursing experience in an acute care, home health, community health, hospice, or related setting.
- Strong clinical assessment and care coordination skills.
- Excellent communication and interpersonal abilities.
- Valid driver's license, reliable transportation, and willingness to travel throughout the service area.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Ability to meet the physical requirements of the position, including lifting and transferring patients as needed.
- Availability to participate in a rotating weekend schedule.
