The Home Health Aide provides essential care and support to elderly patients, ensuring their comfort and well-being. This role involves monitoring vital signs, assisting with mobility, and delivering compassionate patient care. The aide works under the supervision of a Nurse Supervisor and may occasionally accompany residents to doctor's visits. Flexible shift arrangements include both full-time and part-time options.
Responsibilities
- Accompany patients to their doctor visits, carry charts, take notes, and push wheelchairs.
- Assist with patients' personal care needs, i.e., personal hygiene, etc.
- Document services provided in an accurate and timely manner in accordance with company policy and personal care plan. Maintain patient confidentiality at all times.
- Report any changes in the patient’s condition or incidents to the RN Supervisor or therapist on time.
- Carry out other duties that may be assigned
- Assist patients with mobility and daily activities
- Maintain clear communication with patients, families, and medical staff
- Manage time effectively to meet patient care schedules
Required Qualifications
- Minimum 1 year of experience in home health care
- High school diploma or equivalent
- Valid Home Health Aide (HHA) certification
- Proficiency in basic patient care and mobility support
- Strong communication skills with patients and healthcare team
- Effective time management abilities
- Empathy and compassionate patient interaction
