We are seeking a compassionate, organized, and experienced Case Manager to join our team. The ideal candidate will play a key role in coordinating care plans, ensuring smooth transitions, and advocating for the best possible outcomes for our residents. Also, serves as liaison between resident, physician, care team members, and payor by coordinating, monitoring and communicating resident’s progress and cost evaluation and assisting with coordination to the next level of care.
Key Responsibilities:
Coordinate resident care plans in collaboration with nursing and interdisciplinary teams
Serve as liaison between residents, families, and healthcare providers
Manage discharge planning and referrals
Ensure compliance with state and federal regulations
Maintain accurate documentation and case records
Qualifications:
Valid RN WA License
Previous case management experience in long-term care or SNF setting preferred
Strong communication, organizational, and problem-solving skills
Knowledge of Medicare/Medicaid regulations and MDS is a plus
