Job Summary
The NN team is responsible for collaborating with various stakeholders, including local social services and healthcare providers, mental health and substance use treatment facilities, legal agencies and professionals, housing department, and faith-based leaders to identify and link members of the community looking to achieve wellness in their neighborhoods to NN locations. The Neighborhood Nursing Program Manager provides oversight of the operation of Neighborhood Nursing Community hubs, stakeholder engagement, reinforcement of Standard Operations, staff documentation, record auditing, and reports.

Duties and Responsibilities
The Primary Responsibilities Of This Position Are
Administration
- Participation in local/state meetings, conferences, community meetings
- Collaborate, communicate and coordinate follow up care with Neighborhood Nursing team.
- Provide oversight, direction, and leadership to the Neighborhood Nursing Program staff to ensure that standards of practice are met during encounters with clients, families, and the community.
- Provide supervision including recommendations for hiring, training, evaluation, monitoring of staff performance, and discipline.
- Monitor staff documentation, performance goals and standards, and complete monthly, quarterly and annual reports.
- Timely data input and documentation in electronic health records and other data management systems.
- Document and record outcomes.
- Create and produce reports on demand.
- Create and promote marketing materials for the Neighborhood Nursing hubs and locations, East Baltimore, Somerset County and Suburban locations.
- Initiate and assist in developing strong community relationships, attend community meetings and other activities that cultivate trust.
- Locate and establish community hub sites for the Neighborhood Nursing team(s)
- Complete payroll and monitor staff scheduling including field activity, scheduled time off, unscheduled leave, etc.
- Additional duties as assigned.
Education, Experience, Certifications and Licensures:
- Bachelor's Degree required
- Master’s degree, professional preferred
- Experience with medical case management and coordination of medical services to vulnerable populations.
- Direct experience and understanding of the targeted communities within Baltimore City, Somerset County, and suburban areas within Maryland.
- Supervisory experience required.
- Strong computer literacy skills, specifically with MS Word, Excel, Outlook, and SharePoint (365).
- Organizational skills with the ability to meet a demanding workload.
- Detail oriented to complete requirements of files and contract compliance.
- Sensitivity to cultural and socioeconomic characteristics of population served.
- Excellent oral and written interpersonal and communication skills required.
- Critical thinking skills.
- Proven ability to work efficiently and meet deadlines.

Salary : $76,500 - $78,500
