- Participating in multidisciplinary rounds and collaborating with healthcare providers, therapists, pharmacists, and social workers.
- Supporting outpatient services through referral management, transition of care, and home healthcare coordination.
- Supporting utilization review, ensuring medical necessity, reviewing charts, and working with insurance payors for billing and denials.
- Graduated from an accredited nursing program with at least 1 year of relevant experience.
- Current RN license in the relevant state and CPR/BLS certification.
- Competent in electronic medical records (Cerner/Oracle), Microsoft Office, and electronic faxing/scanning systems.
- Strong communication, care coordination, and multitasking skills.
- Preferred: Bachelor's Degree in Nursing, bilingual abilities (English/Spanish or other languages), and prior care management experience.
- Positions involve primarily in-person inpatient and outpatient work, with some remote options after onboarding.
