Role: Nurse Practitioner, Advanced Practice Provider

Responsibilities

• Provide effective, equitable, value-based primary care to adult Medicare patients.
• Conduct office visits for routine and acute issues, including wellness visits and health risk assessments.
• Collaborate with a care team and coordinate with specialists, testing facilities, and agencies.
• Lead population health initiatives, ensuring preventive screenings and care management.
• Assist with patient education, phone triage, outreach, and conduct home visits as needed.
• Participate in promotional activities and other assigned duties.
  
  

Qualifications

• Master’s or Doctorate in Nursing or NP Certification, with active state license and DEA registration.
• Certification in Family, Adult-Gerontology, or Gerontological Nurse Practice.
• Experience in primary care, geriatrics, or family medicine is preferred.
• Knowledge of HCC documentation, ICD-10 coding, and health assessments is advantageous.
• Strong teamwork, communication skills, and passion for population health are valued.
  
  

Additional Details: This full-time role involves 40 hours weekly, offering a comprehensive benefits package including medical, dental, vision, retirement, and wellness programs.
