Role: Hospital Community RN Liaison

Responsibilities

  • Coordinate safe transitions of care between hospital, home, and community settings
  • Perform nursing assessments and develop individualized care plans
  • Educate patients and families on treatment protocols, medications, and follow-up care
  • Collaborate with physicians, social workers, and community providers to ensure comprehensive patient support
  • Reduce hospital readmissions by connecting patients with essential resources and services

Qualifications & Skills

  • Registered Nurse license
